Health History and Vaccinations

Please remember to complete your Health History Form (sent in a separate email), including documentation of a physical with your primary care provider (mandatory) and Medical Provider Request & Consent for Continuity of Medication Management (optional) by July 15th, 2023. You must send in your Health History Form so that the Health Clinic is informed of any  chronic care conditions (i.e.,  Diabetes, Crohns, Epilepsy etc.) Please upload these to Medicat, MICA's secure online student portal, found on the Student Health Center's webpage. If you are living on campus, please ensure that we either have verification of your meningitis vaccination or meningitis waiver form. The State of Maryland requires confirmation of vaccination or completion of the waiver form. If our office does not have verification of these documents, you will not be allowed to move into your room. For answers to questions about the vaccination requirements, medical care on campus, or the health history form, please call the Student Health Center at 410-225-4118. 

For Fall 2023, MICA has instituted a mandatory COVID-19 vaccine policy for all students (sent in a separate email). Students will not be allowed to access campus without proof of vaccine or a completed medical or religious waiver. These documents can be uploaded to the Medicat student portal found on MICA’s Student Health Center webpage.  

Health Insurance

Students are automatically enrolled into the MICA-sponsored student health insurance program but can choose to waive enrollment with proof of comparable health insurance by September 12, 2023. Any waivers requested after September 12, 2023, will be assessed as a financial penalty. You will receive information via email about how to opt out of the insurance plan over the summer. If you are an international student, you will be auto-enrolled and cannot waive.

Health Care Services

Student health services for MICA students are provided in the Wellness Center (410-225-4118), located at 1501 West Mount Royal Avenue on the 2nd Floor. This is in the same building as the school store (back entrance), The Fitness Center, Student Counseling, and Campus Safety. Every MICA student pays a mandatory health fee to access student health services.

Students in need of emergency care should dial 911 for assistance.
