This is a reminder that Day 1 of MICA's annual Flu Shot Clinic for Faculty and Staff is tomorrow Thursday, October 14, 2021

• Location 1: Brown Center, Leidy Atrium from 9:00 AM – 12 noon.
• Location 2: Lazarus Center Auditorium (North Avenue) from 1:00 PM – 4:00 PM.

In order to ensure that Safeway has an adequate number of flu vaccinations available, please RSVP here. 

Please remember to bring the required documents with you:

• NEW: Completed Pre-Screening/Consent form 
• Valid Faculty/Staff MICA ID card.
• CareFirst health insurance card (if applicable).
• Health insurance card from a non-MICA health insurance plan (if applicable).

Non-MICA health insurance plans must cover flu shots at 100% in order to be accepted. Safeway staff cannot process payments for health insurance plans that require a co-pay.

If you do not have the required documents, Safeway staff will not be able to administer the vaccine.

If you are an employee NOT covered under a health insurance plan, MICA will cover the cost of your flu vaccine at no cost to you. You must show your MICA ID in order to have this cost covered by MICA.
